When a Family Needs a 24-Hour Caregiver
A 24-hour caregiver provides uninterrupted care for an elderly or sick person day and night, and rotating specialists keep the quality of care high for months. Families most often come to us when a relative cannot be left alone after a stroke or surgery, a person with dementia is restless at night, a bedridden patient needs constant attention, or the main caregiver is going on holiday. The family needs every shift partner to be equally attentive and to follow the same care rules. That is why we select caregivers with experience working in a team and help set up a clear handover routine.
In the first weeks of recovery it is unsafe for the person to be alone: they need help with every movement and round-the-clock monitoring.
With dementia or pain, the client often wakes at night. A night caregiver keeps them safe and lets relatives sleep.
Repositioning, hygiene, feeding and pressure sore prevention are needed around the clock — a 24-hour rota makes this possible without overloading one person.
When the permanent caregiver is on holiday, sick or off duty, we find a replacement so care is never interrupted.
Caregivers for 24-hour and round-the-clock care
Candidates experienced in round-the-clock care, shift rotas and working alongside a shift partner. Everyone has been interviewed and references checked.
24-hour caregivers for families in Europe, the UAE, the USA and beyond
We organise 24-hour care wherever the client lives: one caregiver on shifts or a team of shift partners, so care continues through weekends, holidays and time off. We take into account local labour rules on shift length and rest, the language of communication, visa matters and a reasonable commute to the client’s home.
What a caregiver does during a shift
Over 24 hours the caregiver provides the full cycle of care, day and night. The exact list of duties is agreed with the family and follows the doctor’s instructions.
- Morning and evening hygiene, help with dressing
- Medication strictly on schedule and as prescribed
- Monitoring blood pressure, pulse and blood sugar, keeping a care log
- Preparing and serving meals according to the diet, help with feeding
- Walks, exercises and manageable activity
- Accompanying the client to doctors and procedures
- Keeping the client’s room tidy, laundry and bed linen changes
- Night duty: help when waking, going to the toilet, changing linen
- Repositioning bedridden clients to prevent pressure sores
- Watching over the client during restlessness, confusion or pain
- Night-time medication and wellbeing checks
- Contacting the doctor and family immediately if the condition worsens
- A detailed handover to the next caregiver: the past 24 hours, prescriptions, changes
- A weekly report to the family on the client’s wellbeing
Additional responsibilities such as travel assistance or extended household tasks are discussed individually and outlined in the agreement.
Why choose Royal Keeper when hiring a caregiver
We help families skip months of searching and connect directly with vetted caregivers — locally and worldwide
Royal Keeper has a large private database: we have been working since 2000, and many caregivers deliberately do not post their CVs online or enter the open market. You gain access to professionals impossible to find through regular listings.
When urgent care is needed for an elderly family member, independent searching takes valuable time. We maintain a ready pool of vetted caregivers and can present suitable candidates promptly.
Should a caregiver need to leave, we handle the replacement process seamlessly. Our agency ensures minimal disruption to the established routine and emotional well-being of the elderly person.
When relocating abroad it becomes difficult to identify caregivers who understand local medical regulations, visa requirements and cultural expectations around elder care. Royal Keeper maintains an international network and selects professionals who can relocate with the family while meeting specific health and legal standards.
Without thorough background checks families may entrust elderly relatives to individuals lacking medical training or verified experience. Royal Keeper maintains detailed candidate histories, verifies references and excludes profiles that do not meet professional geriatric care standards.
Families seeking caregivers often face uncertainty about qualifications and long-term reliability. Royal Keeper handles verification and selection so you can focus on your loved one’s comfort rather than administrative concerns.

How we recruit staff
We discuss the family's lifestyle, schedule, level of responsibility, and service expectations — not just a list of duties.
We record all key collaboration terms in an individual contract. After signing, the team launches the project.
We run an extensive search, interview candidates, and verify their experience, references, and work history.
You receive a shortlist with detailed profiles and recruiter notes; interviews are arranged with selected candidates.
We help agree on terms, support onboarding, and remain available for any questions after the candidate starts.

About Us
Royal Keeper is an international agency for the selection of premium household staff and nannies for families around the world, founded in 2000. For over 25 years we have combined luxury service standards with a very attentive approach to each family.
Our recruitment team speaks several languages and works with clients across Europe, the UK, the USA, the Middle East and Asia, so we understand the cultural nuances and requirements of different countries.
We build long-term relationships: most families come back to us or refer their friends, and candidates bring their colleagues and acquaintances.
Service pricing and cooperation terms
Ranges from 12% to 22% of the specialist’s annual salary and depends on the country of recruitment, the level of the position, the urgency of the search, and the selected contract format. The replacement guarantee period ranges from 6 to 12 months, and the number of replacements depends on the selected contract.
Payment is made in two stages: an initial deposit is required to confirm the order and start work on the request, and the remaining balance is paid after the candidate has been approved.
Special conditions are available for returning clients, repeat requests, and orders involving the selection of several specialists for one family.
24-hour
caregiver
A 24-hour caregiver is the right format for families who need care for an elderly or sick person around the clock but do not want one specialist living in the home permanently. The caregiver works a 24-hour shift and is then replaced by a colleague, so the client is never left alone day or night, while the caregivers get proper rest and stay attentive. Round-the-clock care is especially needed after a stroke or surgery, with dementia, for bedridden patients and whenever the condition may change at any moment.
Royal Keeper selects caregivers for 24-hour care with medical training and experience with elderly and seriously ill people. We can find one caregiver for an alternating-day schedule, or a team of two or three shift partners who coordinate with each other, keep a shared care log and follow one handover routine. The caregiver follows the doctor’s instructions, manages medication and health monitoring, and provides hygiene, meals, night-time safety and accompaniment to appointments. A caregiver does not replace a doctor.
Every candidate is vetted: interview, education documents, references from previous employers and a background check. We help the family agree a shift rota in line with local labour law, draw up the contract and arrange cover when a caregiver is on holiday or unwell.
We place 24-hour caregivers in Germany, Israel, the UAE, the USA and other countries. If you prefer a specialist who lives in your home permanently, see our elderly caregiver page.
